About you in the role
As a Peer Support Worker – Social Prescribing, you will play a key role in advocating for consumer choice and supporting individuals through tailored social prescriptions. You’ll work within the Commonwealth Psychosocial Support – Social Prescribing team to assess needs, connect individuals with community resources, and promote well-being.
