War Memorial Hospital (WMH) is a 3rd Schedule Aged Rehabilitation hospital, situated in Waverley, owned and operated by Uniting and part of the South Eastern Sydney Local Health District (SESLHD). Services are comprised of 24 hour inpatient care, Monday to Friday outpatient departments, several specialist community teams inclusive of Transitional Aged Care, Aged Care Assessment Team, Geriatric Flying Squad, Young Onset Dementia, 7 day a week day centre, 24 hour accommodation service for regional guests and the full range of hospital support services. It is set within beautiful tranquil grounds and is a well-supported team environment.

The Transitional Aged Care (TAC) program provides a temporary package of multidisciplinary support and low level therapy to both community and residential clients. TAC is a goal oriented, time-limited program (upto 12 weeks)  that targets older people at the conclusion of a hospital episode, who require more time and support in a non-hospital environment to complete their restorative process, optimise their functional capacity, finalise and access their longer term care arrangements. The TAC team comprises of Social Workers, Physiotherapists, Occupational Therapists, Nurses, Dietician, Allied Health and Assistant in Nursing care assistants. The TAC service is delivered according to the Department of Health and Ageing and Ministry of Health Program guidelines.

Selection Criteria: 

  • Degree or post graduate qualification in Physiotherapy and current registration with the Australian Health Practitioner Regulation Agency
  • Minimum of 2 years’ experience working as a physiotherapist in a hospital and / or community setting. Experience in aged care/rehabilitation/community/relevant specialty with a high level of knowledge and expertise in contemporary practice highly desirable.
  • Demonstrated ability to provide comprehensive client centred assessment, outlining clinical reasoning, care plan development and participate in case management - in consultation with older people with complex needs, carers and multidisciplinary team (MDT).
  • Demonstrated ability to work effectively both autonomously and within a MDT
  • Proven high-level interpersonal written and verbal communication skills, with ability to communicate across all internal and external stakeholders
  • Demonstrated capacity to plan, implement and evaluate physiotherapy services, with keen interest in quality improvement processes
  • Advanced computer skills including Microsoft Office suite and relevant health databases
  • Current Class C NSW Driver’s license and willingness to drive as part of your role
  • This position is a permanent full-time role working Monday to Friday.
