What you’ll be doing in this role

Our Uniting Recovery program is a lead provider for many community service programs across Sydney which support people to lead fulfilling and independent lives through the delivery of services, education and training which are person-centred and recovery focused.

The Support Worker role with Psychosocial Services is responsible for supporting people who are aged 16+ with severe often episodic mental illness access appropriate and timely psychosocial supports. As a Support Worker, you will coordinate access to holistic supports including assistance with testing for National Disability Insurance Scheme (NDIS) eligibility. The Support Worker will also ensure that supports are provided within a Recovery Framework and work alongside the consumer in the achievement of their goals, in line with their Individual Support Plans.

Your main responsibilities will include:

  • Assist existing consumers of the Continuity of Supports Program transition to the Commonwealth Psychosocial Supports Program.
  • Work with individuals accessing the program using the recovery informed framework to identify consumer strengths and goals.
  • Conduct needs assessments of consumers within the program.
  • Test National Disability Insurance Scheme (NDIS) eligibility criteria for new consumers.
  • Provide psychosocial support services to participants while supporting and assisting people to test their eligibility for the NDIS.
  • Develop, implement, and monitor Individual Support Plans with the consumer, ensuring they are reflective of the consumer’s preferences, needs, goals and abilities.

Who we’re looking for

  • You'll have Certificate IV in Community Services, Mental Health, Youth Work, or equivalent and relevant to the field of work.
  • Ideally, you'll have three (3) experience in case management and developing and reviewing client centred support plans.
  • You'll have working knowledge of the National Disability Insurance Scheme (NDIS), including supporting clients to submit applications to test eligibility for the NDIS.
  • You'll have the ability to provide individualised psychosocial support to people living with severe and persistent mental illness. 
  • You'll have knowledge of the Principles of Recovery Orientated Mental Health practice.
