About the role

- This position is a fulltime fixed term contract up until June 2022
- Flexibility to WFH and be based across various locations on the NSW North Coast

 

The role as C2bMe@Home Area Manager will provide independent provision of comprehensive, evidence-based clinical care coordination, psychological and psychosocial interventions consistent with the model of care. As the Area Manager of Mental Health and Wellbeing, you will be responsible for the setup of individual and group-based therapy, liaising with GPs, home care providers, LHD, local community organisations, and families and carers. You will support consumers in their psychological recovery from isolation aiming to optimise functioning to continue living with dignity.


Duties and Responsibilities include but are not limited to:

  • Manage provision of services within the program, including program coordination and case management.
  • Early engagement and relationship building with stakeholders including GPs, Older Persons Mental Health Network, Home Care organisations and community based and controlled organisations including culturally specific services.
  • Act as mental health “triage” including administering pre-clinical psychological assessment measures, review, and evaluation of client files/incident reports, and staff supervision and support.
  • Manage the day-to-day operations of the programme, monitoring activities against the programme workplan to ensure contractual obligations are met
  • Provide clinical and corporate leadership team members by fostering open communication, establishing collaborative goals and facilitating professional development opportunities for team members.
  • The “Continuing to be Me” C2bMe@Home program is a comprehensive in reach person centred model of care designed to reduce the impact of physical distancing restrictions associated with Covid-19 on the mental health and well-being of older people living in the North Coast of NSW region

    The C2bMe@home model uses trauma informed practice to reinforce identity and purpose, increases connectedness, develops hope, reduces symptomatology, and optimises functioning so that Older People can continue to live with dignity in the community. It offers low and medium intensity interventions through therapeutic groups & individual clinical care led by skilled health professionals.

     

     Essential Criteria:

  • Registered Psychologist and full membership with AHPRA and certified board approved Supervisor or meets supervision criteria
  • Minimum 5 years of clinical expertise and 3 years of clinical leadership experience
  • Proven expertise in the application of evidence based clinical practice in delivering mental health interventions for older people including case management and therapeutic care
  • Experience in the coordination of individualised psychological and recovery-based interventions for older people based on a case formulation approach
  • Demonstrated ability to supervise, lead and manage a clinical team with the ability to exercise a high level of professional judgement when performing a range of complex critical tasks. 
  • Demonstrated ability to allocate resources, set priorities and ensure budgets are met within the services to deliver strategic outcomes
  • Demonstrated experience in developing and maintaining relationships with stakeholders including GPs, LHD, community groups, consumers, families and carers.
  • Mandatory Influenza vaccine compliance
