headspace Mount Druitt, Parramatta and Penrith deliver headspace Early Psychosis, of Uniting - Recovery stream.  It is a specialist service which focuses on providing comprehensive services to support young people who are at risk of developing a psychotic illness, or those who have experienced a first episode of psychosis. headspace Early Psychosis aims to provide early intervention services to young people with emerging serious mental illness, and their families, that are youth-friendly, accessible, consistent, and seamless, which involves ongoing care and support.

Role Overview

The Continuing Care Team (CCT) case manager role involves the provision of comprehensive, evidence-based case management consistent with the EPPIC (Early Psychosis Prevention & Intervention Centre) model of care.

This CCT Case Manager aims to support the treating team to make the health system more accessible for young people registered within the service by assisting with ensuring young people are linked into all appropriate services as required.  The role provides support by way of information, resources and pathways, assisting young people to gain access to services in line with care plans.

Essential Criteria:

  • Certificate IV in Mental Health, Community Services, or other relevant qualification
  • One year or more experience working within clinical, health and/or welfare service and support systems.
  • Experience working with a diverse range of health professionals and young people.
  • Ability to complete referral forms, needs assessments and outcome measures for young people.
  • Understanding of mental health legislation.
  • Ability to work independently as well as collaboratively with the multidisciplinary team.

Even better:

  • Experience or training working with individuals with psychosis
  • ASIST training and/or experience in suicide intervention
  • Mental Health First Aid training
  • Current First Aid certificate
