headspace Parramatta, Mount Druitt and Penrith deliver the headspace Youth Early Psychosis Program. The headspace program offers an “open door”, easily accessible, early intervention service for young people wishing to obtain support for mental health or substance use issues. The headspace Early Psychosis program is a specialist service which focuses on providing comprehensive services to support young people who are at risk of developing a psychotic illness, or those who have experienced a first episode of psychosis.
Role Overview:
The CCT Case Manager aims to support the treating team to make the health system more accessible for young people registered within the service by assisting with ensuring young people are linked into all appropriate services as required.
The role provides support by way of information, resources and pathways, assisting young people to gain access to services in line with care plans developed by CCT Clinicians and the treating team. The CCT Case Manager will use their ingenuity and broad knowledge of local mental health services in the community to ensure that young people receive appropriate supports in a timely manner.
